Botox vs. filler: What's the difference and what do I need?

The most important difference at a glance
One of the most common questions in our practice: “Do I need Botox or filler?” The answer is easier than you think — both treatments have completely different functions and often complement each other perfectly.
Botox: For dynamic wrinkles caused by facial expressions
Botulinum toxin specifically relaxes the muscles that cause wrinkles. Perfect for:
- forehead wrinkles — horizontal lines on the forehead
- Wrath Lines — vertical wrinkles between the eyebrows
- crow's feet — Laugh lines at the corners of the eyes
- special applications — Lip flip, gummy smile, teeth grinding
The treatment takes effect after 3-5 days and lasts about 4-6 months. The result: relaxed, smooth skin with natural facial expressions.
Filler: For volume and static wrinkles
Hyaluronic acid fillers fill in volume losses and smooth out deep, existing wrinkles. Perfect for:
- lips — natural enlargement and contouring
- nasolabial wrinkles — wrinkles from the nose to the corners of the mouth
- Puppet pleats — from corners of mouth to chin
- Cheeks & temples — Volume building in the face
- hands — Rejuvenating the back of the hand
The effect is immediately visible and lasts 6-12 months, depending on the preparation used and the treatment area.
When do I need something?
You need Botox if:
- Wrinkles are only visible with facial expressions (laughing, frowning)
- You want to take preventive action against wrinkles
- Your forehead, eye area, or frown line is affected
You need filler if:
- Wrinkles are visible even at rest
- Your face has lost volume
- Lips, cheeks, or deep wrinkles should be treated
Often the combination is perfect
In many cases, we recommend a combination of both methods. For example: Botox for the forehead and eye area, filler for nasolabial wrinkles. This is how we achieve a harmonious, natural overall result.
